  If you have an upcoming appointment scheduled at our office or if you'd like to become a new patient, please click the link below to access our new patient packet. 


We understand that an abundance of paperwork may seem overwhelming. Submitting it online is secure and will save you valuable time during your visit.

Want to socially distance but need an appointment? 

Have to work, but need medication refills? 


Telehealth services are available. 


All you need is a computer or smartphone with internet access to get started. 


How does telehealth work?  We use a HIPAA-compliant application called doxy.me to provide telehealth services. It is easy to use from a computer, tablet, or smartphone with internet or cellular access. 

1. Please click your provider's name below at your appointment time. 

2.  Choose 'yes' when the app asks to access your camera and microphone (otherwise we won't be able to see or hear you). 

3. Enter your FIRST and LAST NAME when prompted. You will be placed in a virtual waiting room queue.

4. Sometimes you may have to wait. If you feel you have been waiting too long please access the chat feature and message us. Please do NOT hang up and log back in repeatedly as this places you at the bottom of the queue.
